What does it do?

CONNECT CLINIKO WITH GRAVITY FORMS

The Cliniko Add-On for Gravity Forms lets you create patient records and manage appointments whenever a form is submitted on your WordPress site.

Form submission data is automatically sent to Cliniko through their API. Configure your integration once and let it run automatically on every submission.

How does it work?

After installing, navigate to your form’s Settings tab and select Cliniko. Enter your Cliniko API credentials to connect. Then create a feed to define how form fields map to Cliniko data.

How do I enable this?

Install and activate the Gravity Forms Cliniko Integration Add-on on your WordPress website.

Navigate to Forms → Settings → Cliniko, enter your Cliniko Shard (region) and API Key.

Open your patient registration or appointment booking form, go to Settings → Cliniko, and click 'Add New'.

Select your Action (Create Patient or Book Appointment) and map form fields to Cliniko fields.

Save the feed, enable conditional logic if needed, and submit a test entry to verify live patient intake.
