Gravity Forms
+ 360 Feedback & Peer Reviews

Run 360-degree feedback and peer reviews with Gravity Forms. Set up a review cycle, add the people being reviewed and their raters in groups (self, manager, peers, direct reports, external), or let each person suggest raters for your approval. Every rater gets a signed personal link to your Gravity Forms questionnaire that works once, with reminders and a deadline. Answers are stored apart from the person who gave them, a group's results stay hidden until it reaches the minimum number of responses you set, and comments are shown without names. Each person gets a clear report with averages per question and group, the gap between their self-rating and everyone else, how answers were spread, and the comments, as a page and a PDF. A dashboard shows response rates, and the CSV export holds aggregates only. All processing is local, with no outside connection.

What does it do?

A 360 review asks several people who work with someone (their manager, their peers, the people who report to them, sometimes clients) the same questions, and puts the answers next to the person's own self-assessment. Done with a plain form it falls apart quickly: you need a personal link per rater, you need to know who has not answered yet, and above all the answers must stay anonymous, or nobody will be honest. The Gravity Forms 360 Feedback & Peer Reviews add-on turns any Gravity Forms questionnaire into a complete multi-rater review.

You create a review cycle, choose your questionnaire form and a deadline, add the people being reviewed (WordPress users or a pasted list) and their raters in groups: Self, Manager, Peers, Direct reports and External. Each person can also suggest their own raters on a personal page, with your approval. Every rater receives a signed one-time link; a changed, replaced or reused link is refused, and reminders go only to those who have not answered.

Anonymity is built in. Answers are stored apart from the rater: only a completion flag records that someone answered. A group's results appear only when it reaches the minimum number of responses you set (for example 3), on the report page, in the PDF, in the CSV and in the report e-mail. Comments are shown without names and in random order, and the person reviewed never sees who their raters were or who answered. Each person gets a report with averages per question and group, the gap between their self-rating and everyone else, the spread of answers and the comments, as a page and as a PDF. Everything runs on your own server, with no outside connection. For Gravity Forms 2.9 and newer.

How does it work?

Each review cycle stores its people, raters and answers in the add-on's own database tables. When you send invitations, every rater gets a link that carries a random code and a signature made with a secret key of your site; only a fingerprint of the code is stored, so a link that was changed, copied to another rater or replaced by a newer one is refused before anything is read. The questionnaire page shows your Gravity Forms form only with a valid link. When the rater submits, Gravity Forms validates the answers as usual; the add-on then checks the link again, marks the rater as done in a single database step (so the same link can never be used twice) and saves the rating and comment answers under a random ID together with the cycle, the person reviewed and the rater group, and nothing else. The Gravity Forms entry is removed straight away, the IP address is not stored and the form's own notifications are not sent, so no record says who answered or when. Reports are calculated when you open them: a group is shown only when it has reached its minimum number of responses, hidden answers are left out of every number, including the All others average, and comments are shown without names in an order that has nothing to do with when they arrived. The PDF is drawn on your server by a bundled PDF library. Everything happens on your own server — the add-on makes no outside HTTP request beyond the licence and update checks.

2
Build your questionnaire in Gravity Forms: rating questions as Radio Buttons (1 to 5) and comment questions as Paragraph Text.
3
Open Forms → 360 Feedback, create a review cycle, pick the form, the deadline and the minimum responses per rater group, then add the people to review and their raters.
4
Open the cycle and send the invitations. When the cycle closes, read each person's report on screen or as a PDF and share it with them.
