Turn a form submission into a signed document. The Gravity Forms DocuSeal Premium Add-on creates a signing submission from one of your DocuSeal templates when a form is submitted, and sends the submitter a signature request by email — with the name, company and other details they just entered already filled in. Everything runs through your own DocuSeal API key, so your documents stay in your account and nothing passes through GravityExtra.

The add-on works with both DocuSeal cloud and a self-hosted DocuSeal server: leave the base URL blank for the cloud, or point it at your own installation to keep signing entirely in-house. Pick a template from a list read from your account, map the signer's email, name and role, and prefill any template fields from form fields or merge tags. Signature and initials fields are always left for the signer.

Your API key is encrypted before it is stored on your site, it is never shown again and never written to an entry or a log, and the add-on only ever contacts the DocuSeal host you configured. A failed call never blocks the submission: the entry is saved and a note records the reason, so no lead is ever lost to a hiccup at the API.

On submit, the add-on creates a submission from the chosen template with the signer and the prefilled values you mapped, using your own API key against the DocuSeal host you configured (cloud or self-hosted). DocuSeal emails the signer a link to sign, and the add-on writes a note on the entry with the masked signer email and the submission number. The API key is encrypted at rest and the host is the one you set, so nothing is sent anywhere except your DocuSeal.
