Bring your website feedback into Formbricks. The Gravity Forms Formbricks Premium Add-on creates or updates a Formbricks contact from every submission, with the email, user ID, first and last name and any attributes you map from your fields, and can record the answers as a response of the Formbricks survey you choose.

It works with Formbricks Cloud and with your own self-hosted Formbricks. Enter the address and an organization API key, click Test connection to list the workspaces the key may use, and pick a default workspace. Each feed decides the workspace, how the contact is filled, which survey receives the response and how each question is answered, with merge tags, checkbox lists, hidden fields and conditional logic.

Returning visitors are matched to their existing Formbricks contact by email, so there are no duplicate profiles, and a retry or Resend never records a second response. Failed calls that may pass are retried automatically, notification events tell your team when sending failed, and your API key is encrypted on your site, never shown again and never logged. A failed call never blocks the submission: the entry is saved and a note explains what happened.

When a form is submitted, the add-on reads the attribute keys of your workspace, then creates or updates the contact with PUT /api/v2/management/contacts/bulk, which Formbricks matches by email. If you chose a survey, it records the answers with POST /api/v2/management/responses and a single-use ID that Formbricks keeps unique per survey, so a retry can never record twice. Every call uses your API key in the x-api-key header over https, never follows a redirect and only reaches the Formbricks address you set.
