Site walk-throughs, vehicle checks, cleaning rounds, equipment audits and move-in reports all follow the same pattern: a list of things to look at, a verdict for each one and proof of anything that is wrong. The Gravity Forms Inspection Checklist add-on turns that pattern into one field. You build the checklist once, in sections and items, and the inspector works through it on a phone with big Pass, Fail and N/A buttons.

When an item fails, the field can ask for a photo (straight from the phone camera, made smaller before upload), a note and a severity such as Minor, Major or Critical. Section progress, a jump to unanswered button and a sticky score bar keep the walk-through moving. The score is the share of passed items among the items that apply (N/A does not count), by count or by weight, and a pass mark turns it into Pass or Fail. A critical failure can fail the whole inspection on its own.

Everything is checked again on the server when the form is sent: required answers, the photo or note required on a failure, the severity and the score itself, so the result in the entry is the one your rules produce. Each entry shows a clear report with photo thumbnails, merge tags put the score, the result and the list of failed items with notes and photo links into your notifications, conditional logic can react to the result, and the entry export has a column for every item and note. The add-on helps you record inspections; the rules you inspect against remain your organisation's responsibility. Everything runs on your own server. For Gravity Forms 2.9 and newer.

The field stores your checklist in the form: sections, items and the options of each item. On the page every item is a card with Pass, Fail and N/A buttons; failing an item opens its severity, note and photo boxes. Photos are made smaller in the browser before they are sent, then checked on the server (image type, size limit, the Gravity Forms list of blocked file types) and kept in the form's protected upload folder; links to them go through the Gravity Forms secure download handler. When the form is sent, the add-on reads the answers, checks every required answer and the photo, note and severity a failed item needs, and recalculates the score and the result from your settings. The values posted by the browser are never trusted. The entry keeps a snapshot of the inspection (item texts, answers, notes, severities and photos), so later changes to the checklist do not rewrite earlier inspections. The score, the result and the number of failed items are stored as separate values, so conditional logic, notifications, entry search and the export can use them. Everything happens on your own server — the add-on makes no outside HTTP request beyond the licence and update checks.
