Not everyone fills in your forms on a screen. The Gravity Forms Printable Blank Form add-on turns any form into an empty paper form that people print and fill in with a pen — at reception, at an event, on a site with no signal — drawn straight from the live form, so the paper version never falls out of date.

Every field becomes the right paper control: writing boxes sized to the field, comb boxes for short codes, ruled lines for paragraph text, a tick box for every choice of radio buttons, checkboxes, drop downs and multi selects, labelled sub-boxes for name, address, date and time, a price with a quantity box for products, and empty tables for list fields. Sections, HTML text, page breaks, descriptions and required markers are kept; hidden, admin-only and calculated fields, CAPTCHA, password and card fields stay off the sheet. Conditional fields, sections and pages carry a plain note such as Only if Contact method is Phone.

Add your logo, title and an intro, choose A4 or US Letter and a font size, and finish with a signature and date block. Print from the form list or the form editor toolbar, or place a download link for visitors on any page with a shortcode — switchable globally and per form, with signed links. PDFs are cached and redrawn automatically whenever the form or the settings change. Everything runs on your own server with a bundled PDF library: no outside connection. The add-on reads the form definition exactly as you built it and lays it out top to bottom on A4 or Letter paper with a bundled PDF library. Each field type is mapped to a paper control: text fields to boxes sized by the field size, short maximum lengths to comb boxes, paragraph text to ruled lines, choices to round or square tick boxes laid out in columns, compound fields to captioned sub-boxes, list fields to empty tables and page breaks to new sheets. Fields placed side by side in the editor share a line when they fit, and a field is never split across sheets unless it is longer than a page. Conditional logic rules are turned into short sentences using the labels and choice texts people see. The finished PDF is cached in a protected folder; its file name carries a fingerprint of the form and the settings, so any change to the form or the settings produces a fresh PDF on the next request. Visitor links are signed and only work while the form, the global switch and the form's own switch allow it. Everything happens on your own server — the add-on makes no outside HTTP request beyond the licence and update checks.
