Short quizzes with a personal result are a friendly way to get people thinking before a workshop, a course or a team day. The Gravity Forms Scored Assessments & Personality Results add-on turns any Gravity Forms form into a scored assessment for self-reflection and engagement.

Set up categories (working styles, skills, readiness areas or types) and give each answer points in one or more categories, for radio buttons, drop downs, checkboxes, multi selects, likert scales and number questions, with weights and reverse-scored statements. Scores per category are shown as a sum, an average or a percent of the possible range, and lead to an outcome: the highest category (with a tie rule), the top categories, low / medium / high bands per category, or your own rules.

Respondents see their outcome with a bar or radar chart right after submitting, plus a signed private results link and a PDF. Outcome and scores are stored on the entry for merge tags, entry list columns and notification and confirmation conditional logic (route follow-ups or redirect by outcome), and an aggregate report shows the outcome distribution and averages per category. Scoring always happens on your server: numbers from the browser are never used. No outside connection. For Gravity Forms 2.9 and newer.

The scoring settings are saved with the form. When an entry is saved, the add-on reads the answers of the scored questions from the entry, adds up the points per category (with weights, reverse scoring and the limits of number questions), turns them into the score you chose (sum, average per answered question, or percent between the lowest and highest possible score) and decides the outcome. Outcome, result title, score and band per category are stored as entry meta, which Gravity Forms shows as entry columns and uses in notification and confirmation conditional logic; a snapshot of the result is kept so the respondent's results page and PDF stay as they were. Charts are SVG drawn by the add-on, the PDF is made with a bundled library, and private links are signed with a secret of your site. Numbers sent by the browser are never used. Everything happens on your own server — the add-on makes no outside HTTP request beyond the licence and update checks.
