“The button on the pricing page looks broken” is hard to act on. Which page, which button, which phone? The Gravity Forms Site Feedback Button & Page Screenshot add-on puts a Feedback button on your site that opens your own Gravity Forms form in a slide-in panel and attaches a screenshot of the page the visitor is looking at, with the technical details your team needs.

Choose the label, colour, icon and position of the button, the pages it appears on (all, only some, or all except some) and the visitors who see it: everyone, logged-in users, chosen roles or administrators only – ideal for client reviews of a site in progress. The panel is keyboard accessible: focus moves into it, stays inside while it is open and returns to the button when it closes.

Add the Page Screenshot field to the form. The screenshot is taken in the visitor’s browser when the panel opens; the visitor can draw boxes and arrows, add numbered pins, blur anything private, retake it or remove it. Password fields, card fields and any element you mark are covered automatically, and embedded frames are never captured. If a page cannot be captured, the visitor is told so and can upload a screenshot instead.

Each entry stores the page URL and title, viewport and pixel ratio, browser and system, the logged-in user, the scroll position and the last 10 JavaScript errors of the page. Query strings are removed from captured URLs by default, so tokens in links are never stored. You see it all in a Feedback page column with a thumbnail, in a Page feedback box on the entry and in {feedback:url}, {feedback:screenshot}, {feedback:browser} and {feedback:errors} merge tags for your team notification. One click creates a ready-made feedback form with that notification. Screenshots are checked and cleaned on your server (PNG only, size and pixel limits, metadata removed), stored with your Gravity Forms uploads under a random name, and submissions are rate-limited. Everything runs on your own server: no outside connection. For Gravity Forms 2.9 and newer.

The button and the panel are added to the pages you chose, for the visitors you chose. When the panel opens, a small script loaded from your own site draws the visible part of the page into an image in the visitor’s browser, leaves out the panel and any embedded frames, and covers password and sensitive fields. The visitor marks the image up; the final picture is sent as a PNG with the form, together with the page details and the JavaScript errors recorded since the page loaded. On your server the add-on checks that the file is a real PNG within the size and pixel limits, removes everything that is not image data, writes it into the Gravity Forms uploads folder of the form under a random name and saves the page details with the entry. Nothing is sent anywhere else — the add-on makes no outside HTTP request beyond the licence and update checks.
