Schools, care homes and workplaces often check in on wellbeing with paper slips or a shared spreadsheet: nobody sees the pattern over weeks, the wrong people can read the answers and a request to talk can wait days in a tray. The Gravity Forms Wellbeing Check-ins add-on moves the check-in onto your own website. People answer a five-step mood scale with emoji and words, optional tags, an optional comment and an I'd like to talk to someone box, through a signed personal link or their own login, weekly on the day you choose or daily on weekdays, with reminders only to people who have not answered yet.

When a score is low twice in a row, someone asks to talk, or a comment contains one of your keywords, a follow-up alert is created exactly once and e-mailed only to the lead of that person's group. Leads acknowledge, add notes and close alerts in an alerts queue, and every step is kept in an audit log.

Group trends show weekly averages and the spread of answers only when at least five people answered, individual histories are visible to the group lead and administrators only, and a support note with your own contacts appears with every check-in. Each person is saved in the add-on's own table with a random reference, and every reminder carries a link signed with your site's secret keys; a link that does not verify, or belongs to someone else, is refused before anything is saved. Each check-in is stored once per person and period with a unique key in the database, so a second one replaces the first and is written to the audit log. After every check-in the add-on compares it with the person's previous one and checks the talk box and your keyword list; each alert is claimed with one insert on a unique key, so it is created and e-mailed exactly once, only to the lead of the person's group. Group trends are calculated from the stored check-ins and shown only when at least five different people answered. Reminders run on the WordPress scheduler and skip everyone who has already checked in. Everything happens on your own server — the add-on makes no outside HTTP request beyond the licence and update checks.
